This document is not a pulp magazine but rather a U.S. Air Force Project 10073 record card for UFO sightings. The form captures data on unidentified aerial observations, requesting location, date-time, observation type (ground, air-intercept, etc.), photographic evidence, observation duration, number of objects, and flight course.
