Β· January 1961

This document is not a pulp magazine, but rather a U.S. Air Force UFO report form (ATIC Form 164) documenting a sighting at Maxwell Air Force Base, Alabama on January 23, 1961. The handwritten entry describes a blue or blue-white, coin-like object observed for two seconds moving east-west or southeast-northwest at an estimated 450+ mph. Multiple witnesses reported the object appeared to "flame out" before reaching the ground, with sighting angles recorded as 180 degrees from true north at 45-60 degrees elevation initially, disappearing at 100 degrees from true north at 30 degrees elevation. The Air Force concluded the phenomenon was likely a meteor entering Earth's atmosphere that burned out in the lower atmospheric layers. The form includes investigative sections for witness details, object characteristics, and comparative observations.
