This document is a Project Blue Book record card (Form 329, revised September 1952) documenting UFO sighting reports. The form captures standardized information about unidentified aerial phenomena, including observation type, location, date-time, and number of objects sighted.
The second sighting section describes a cylindrical mass of flame, approximately grapefruit-sized and orange-colored, with no smoke or flame trail. The object attracted attention while moving plainly across the sky at approximately 65 degrees to the observer's right, flying directly south from a position 25 miles north of Anathan Island, north of Saidan. The object maintained a straight, level flight path consistent with the observer's perspective, then faded out. The observer did not believe the object was obscured by cloud cover, as the sky remained clear and the moon was shining. The object appeared to move in a flat trajectory without banking, eventually fading out as it headed in an easterly direction.
