John Riley, a suicidal doctor hospitalized in White Lake after a barbiturate overdose, is visited by a monstrous creature at his window—a being with wolf teeth, inhuman feet for climbing, and clawed paws. Riley, desiring death, offers no resistance as the creature wounds his throat. A nurse discovers the scene, glimpsing the creature before it falls from the window into the darkness. Police officer Stuart arrives to investigate, skeptical of the nurse's account yet unable to ignore physical evidence: Riley's throat wound and enormous claw marks on the hospital wall.

Stuart learns Riley is a renowned transplant surgeon with a mysterious past. Two years prior, a Mexican maid named Soledad Huarte disappeared from the home of wealthy Patricia Wimbledon; the investigation led to Riley's residence but found no evidence. Stuart begins examining Riley's belongings and requests archived files on the earlier case, suspecting a connection between the vanished woman and the nocturnal attack.
